1.	I understand completely the subject matter and nature of the practices of bondage and discipline, sadomasochism, power exchange between willing and consenting adults, homosexuality, bisexuality, heterosexuality, fetishism and other related sexual or dominance and submission activities.
2.	I further state and agree to the fact that I find none of these subjects or activities to be offensive or objectionable to me in any manner whatsoever.
3.	As a member of The Leather SINS Social Club, NFP, I do not find patently offensive or lewd any act of sexual intercourse, any act of deviant sexual intercourse, insertion of one or more fingers or other objects into the vagina or anus of another human being, masturbation in any form, caressing or fondling of the genitals of another person, representations or imitations of sexual intercourse, masturbatory or excretory functions whether or not accompanied by an exhibition of the genitals, or the fondling in any manner or caressing of the body of another person of either sex by either sex.
4.	I agree that I am joining a private organization, not open to the public. As a member of such organization, I willingly and with full consent and knowledge waive and relinquish any claim, legal or otherwise, that I might make as to the offensive nature of anything I might see or experience while a member of this organization.
5.	Should I at any time attempt to make such a claim, I recognize that I would do so solely as a means of entrapping and/or soliciting the arrest of other members of The Leather SINS Social Club, NFP
6.	I agree not to bring or consume any alcoholic beverage or illegal or illicit substance of any sort to any meeting, seminar or other type of gathering of any sort sponsored by The Leather SINS Social Club, NFP
7.	I will never solicit for, negotiate, or consent to any form of payment for sex during a meeting, seminar or gathering sponsored by The Leather SINS Social Club NFP I am aware that such conduct is illegal under Illinois State Statutes.
8.	Should I violate either paragraph 6 or 7 above, I agree to be immediately ejected from any Leather SINS Social Club, NFP event and to forfeit my membership in this private organization.
9.	I am at least twenty-one years of age. I agree to produce a valid driver's license or other valid photo ID in order to obtain my membership card prior to entering any event sponsored by The Leather SINS Social Club, NFP to prove my age and identity.
10.	The name presented is my true and legal name and not an alias.
11.	I assume the risk of injury incident to my attendance and participation at meetings, seminars and other events sponsored by The Leather SINS Social Club, NFP and I agree to hold harmless The Leather SINS Social Club, NFP, its officers, directors, organizing committee, volunteers, staff and members, as well as the Hotel, the building management, the building owners and their agents, successors, and assigns from any claim of liability by me that may arise out of my attendance and participation at meetings, seminars and other events sponsored by The Leather SINS Social Club, Inc.
12.	I agree not to bring in a camera, a cell phone camera, a cell phone capable of either recording voices or recording any sort of images either digitally or over email or a video recording device into any event sponsored by The Leather SINS Social Club, NFP (including the vendor areas, seminars, demos and the play space). I further agree that should any camera, video recording device or audio recording device be brought to a The Leather SINS Social Club, NFP event by me or by someone acting on my behalf, such equipment will be subject to immediate confiscation and all film, media or recording data will be subject to immediate destruction.
13.	Specific rules which apply to the Play Space are as follows:
¤	No alcohol or any illegal drugs or illegal substances of any kind may be brought into the Play Space. Anyone who is noticeably affected by either alcohol or drugs will be denied admittance or asked to leave by the Play Space Monitors.
¤	All attendees for The Leather SINS Social Club, NFP will be required to present a membership badge and state picture ID for admittance to the Play Space. There will be NO exceptions to this rule. So be sure to bring your badge and ID!
¤	Smoking is prohibited in the active Play Space. Smoking will be permitted only in designated areas outside the Play Space.
¤	Prostitution, solicitation and negotiation of compensation for sexual services are illegal, and will not be tolerated. Any violator will be immediately ejected from the premises and their membership forfeited.
¤	Food and drink (with the exception of water) shall not be brought into the Play Space.
¤	No cameras or cell phones of ANY kind and no video or audio recording devices of any kind will be allowed in the Play Space. (Even if the cell phone is not capable of taking pictures, all cell phones will be banned from the Play Space.) **NOTE: CELL PHONES OF ANY KIND FOUND IN THE PLAY SPACE DURING THE FRIDAY AND SATURDAY NIGHT PLAY PARTIES WILL BE CONFISCATED! Any person seen with any such devices shall be immediately ejected from the premises and their membership canceled.
¤	For everyone's protection, we ask that you use the rubber gloves and cleaning supplies located throughout the Play Space.
¤	Cleaning supplies are available at safety stations throughout the play space and classrooms. If you do a wax or other messy scene, please use a tarp or drop cloth. Please clean up after your scene. Leave all equipment free of sweat, bodily fluids, wax, lubricants, etc. Bring bodily fluid contamination to the attention of a PSM.
¤	Normal Play Space etiquette is required. Do not intrude into a scene unless specifically invited by the participant. Do not touch anyone or anyone's gear without permission.
¤	Watching from a respectful distance is permitted, but please keep all conversation, laughter and comments to a minimum in the Play Space. DO NOT talk to the players while the scene is progressing.
¤	Play Space Monitors must be obeyed at ALL times. If someone is breaching Play Space etiquette and your scene is being disturbed or you are being bothered unduly, please notify a PSM.
¤	Membership Fees will not be returned to anyone removed from any Leather SINS, NFP event for violation of any of the above regulations.
¤	No membership refunds will be issued for any reason after February 1, 2008

HOST HOTEL RESERVATION & CANCELLATION POLICY
While we are not directly booking your hotel reservations, Leather SINS has guaranteed every room in the entire hotel for your enjoyment, safety and security.   We therefore must make sure that you understand the hotel’s cancellation policy for this event.  All reservations will be charged at the time you make your room reservation.  Should you decide to cancel, the hotel will credit one night's room and tax back to you if your cancellation is made before January 29, 2008. Any cancellations made on or after January 30, 2008 will be billed in full for the total cost of all days that you reserved at the hotel.
